It’s been 15 years since the idea of forming a central Illinois primary care physician network affiliated with Memorial Health System was first conceived. We were established as Health Care Network Associates in 1994. What was then a vision has become a stunning success.

We have taken on a new name: Memorial Physician Services. Our primary care network is comprised of over 40 physicians, 10 nurse practitioners and a physician assistant serving patients in 11 locations. We have approximately a quarter of a million clinic visits every year. In 2009, we plan to establish additional clinics and welcome more providers to our network. In addition, we are affiliated with a health system that aspires to be a national leader for excellence in patient care.

While our name has changed, our commitment to high-quality patient-centered care remains the same.
